By JoAnna Voyles (Kapaa, hi United States)
I wish I would have read the reviews before placing the order. Patients were getting the bills instead of the insurance companies.

1.0 out of 5 stars.  HCFA's slide, bad news.., July 07, 2014
By Kari Quickenden
The hcfa forms can slide up and show the individual's address and not the insurance address. So the mail is delivered to an individual and not the insurance company. These are not like the ones that are blue and have an adhesion above the label window to keep them in place.

2.0 out of 5 stars.  Too Big For the Current Form 1500 (2/12), June 11, 2014
By Robert Andres (California)
I suppose it's my fault for not reading the description more carefully, and seeing that these are an inch too tall and half an inch too wide for the current HCFA forms, which are the size of a regular sheet of copy paper. However, this product description was e-mailed to me, with the suggestion that people like me who order the new HCFA forms from Amazon have also ordered these particular envelopes. If that is the case then they too are ordering them in error.

The address will appear too low thus cutting off the bottom line. I can still use these envelopes if I print the insurance company's address on copy paper like a cover sheet and tape it in position. Once this supply is gone I'll be searching for an appropriately formatted envelope.
